What Is a Function Report, and Why Does It Matter So Much?

A Function Report is a form where you describe, in your own words, what your daily life actually looks like now, things like getting dressed, cooking, sleeping, and concentrating. It matters enormously, because it's one of the only places in the entire process where the decision maker hears directly from you.

A common mistake is answering too briefly, almost like a checklist, because it feels embarrassing to admit how hard things have become. Try to avoid that. Instead:

  • Be specific. Instead of “I can cook,” try “I can microwave simple meals, but I can't stand long enough to cook a full dinner.”
  • Describe bad days honestly, alongside your better moments.
  • Explain how long tasks take you now, compared to before.
  • Mention if you need reminders, breaks, or help from someone else.

Think of it this way: nobody watching you live your life gets to see the version of you at 2 a.m. when the pain won't let you sleep, or the third time you had to sit down while folding laundry. This form is your chance to describe that reality clearly and honestly.

Many attorneys review these forms with their clients before submission, which helps make sure nothing important is left out and every answer is consistent with the medical record.
